With our secure, video-enabled technology and integrated diagnostic tools, Sound's tele-hospitalists support a full range of enhanced clinical care, after-hours admissions, and general cross-coverage needs on nights, weekends, and holidays. Services include evaluation and treatment for changes in condition, lab orders and reviews, assessment and treatment for falls, and drug orders or drug clarifications. Along with a daily log of all patient interactions, our tele-physicians document every connect directly into your facility's EMR to ensure a seamless handoff in the morning. With thousands of connects each month, our tele-hospitalists treat 96.6% in place at the SNF.

Emergency Department Transfer Management

When an emergency department (ED) transfer becomes necessary, Sound's tele-hospitalists manage the transfer to support the best outcome for your resident. This includes calling the ED for a peer-to-peer with an emergency room physician to explain the patient's condition and goals of care and expediting a return to the facility as soon as it is safely possible. The results – 33% reduction in hospital readmissions on average for our SNF partners.

PointClickCare Integration

Integration between PointClickCare and Sound's telemedicine video call platform, VCP, provides greater ease of use and efficient workflows for Skilled Nursing Facility staff and Sound's clinicians. Existing and new patient profiles will automatically sync from PointClickCare to VCP, allowing users to easily and immediately begin an on-demand video. Patient profile changes in PointClickCare will update in real-time in VCP, ensuring data parity between systems without manual duplication. Clinician video consult notes will also automatically sync from a patient's VCP profile to their PointClickCare profile.
