HealthShare Exchange

Overview

HealthShare Exchange is a health information exchange (HIE) with more than 400 healthcare organization members across the care continuum. It links the electronic medical record (EMR) systems of many healthcare providers and makes this information accessible at inpatient and outpatient points of care, including post-acute care facilities, home health organization, hospitals, and medical practice offices, for treatment and care coordination purposes. HSX services provide recent clinical information and alerts to members to help inform patient care and improve outcomes.

Features and Benefits

Encounter Notification Service for Facilities

The Encounter Notification Service for Facilities provides notifications to post-acute care facilities about discharged residents who are subsequently admitted within a defined timeframe (e.g. 90 days) to HSX-Member emergency departments, hospitals or other post-acute care facilities. These notifications can provide insight into hospital re-admissions and are valuable for facilities involved in ACOs, preferred networks and bundled payment programs.

Clinical Data Repository

Collects and retains medical-records data about patients, permitting a patient’s provider and/or care-team members to query for clinical information about the patient.

Contributing Clinical Data for HIE

Post Acute Care facilities can contribute data to HSX through the connection with PointClickCare. The data types include admissions and discharges for all patients that receive care at your facility as well as Continuity of Care Documents that include more robust clinical data. Sending this data to HSX as an HIE will allow for more informed transitions of care as patients are admitted and discharged from facilities.
